The person completing and submitting this form should have the authorization to accept the Terms and Conditions and will be responsible for adding or deleting users for your agency. This person is referred to as the "Site Manager". The Site Manager will receive a confirmation email with an administrator user name and password. This login will allow the Site Manager to subsequently create a login for all users in their office.
